Across the health system | A question to investigate
Follow discharge medicines work beyond the send button.
The NHS Discharge Medicines Service connects hospital discharge information with community pharmacy support (NHS England, n.d.). For commissioners and provider teams, the interesting question is what happens along that connection. NEUVIOR's proposed DMS Commissioner Value Scan examines the difference between expected activity and the activity a local pathway can actually show. The aim is a more useful account of referrals, action and completion, so a team can decide where improvement deserves attention before committing resources to an intervention.

Begin with a pathway, not a league table.
A comparison between hospitals or pharmacies is tempting, but numbers can reflect different referral populations, recording practices and local arrangements. We would begin with one agreed pathway and establish what each stage means. Which referrals are in scope? What counts as received, acted on or completed? When is an exclusion legitimate? Answering these questions prevents a missing record from being presented as a service failure and stops a convenient denominator from creating a misleading impression of performance.
Two proportions answer different questions: actioned referrals divided by referrals received, and completed service episodes divided by the agreed eligible, linkable referral cohort. A pathway should not quietly switch between these denominators. Record a separate unknown or unlinked group, and distinguish referrals that have had enough follow-up time from those still progressing. A team can then see whether the apparent gap concerns transmission, action, completion or simply information that is not yet available.
Ask where the value is supposed to appear.
The proposed scan would make the local theory of improvement explicit. Better information transfer might support a clearer follow-up conversation, fewer avoidable queries or more reliable coordination. Those are hypotheses to examine. They should not be converted automatically into avoided admissions or a financial return. A commissioner needs to see which step is observable, who is responsible for it and which downstream claim would require a different study. This keeps the ambition connected to evidence the pathway can realistically produce.
A referral is the beginning.
Follow the public discharge medicines pathway. Select a handover to see what the team needs to know before calling the journey complete.
Discharge referral
The referring team shares the medicines information needed by community pharmacy. Sending and receiving are separate events: the first question is whether the intended pharmacy has usable information.
The useful question
What information is missing, and which team can supply it?
Observe receipt and clarification requests. A sent referral is not a completed service.
Public pathway interpretation | NHS England (2021) | Discharge Medicines Service toolkit. No patient records or operational results are shown.
Use the smallest sufficient information set.
An exploratory scan could start with permitted aggregate counts, definitions, pathway documents and staff descriptions of exceptions. Patient-level material is not needed simply to establish whether organisations use compatible stage definitions. Counts should include the reporting period and coverage. Explain where stages cannot be linked or where reporting is incomplete. This approach gives the team a useful first analysis while allowing it to decide whether a more detailed evaluation is justified and what access would be required.
Compare the scan with current review work.
A bounded PHARMORIS evaluation could compare the team's existing service-review pack with a prepared, source-linked pathway brief. Reviewers would test whether the brief correctly identifies missing stages, denominator problems and ownership questions. Inputs and the rubric would be agreed in advance. The exercise would evaluate analysis quality and preparation burden, not delivery of the clinical service. The original Commissioner Value Scan is a proposed programme direction; it is not evidence of a commissioned pilot, an accepted contract or realised benefit.
Make the pathway visible before assigning value.
A useful handover is information that reaches someone able to act. Explore a constructed case before deciding what to measure.
A commissioner and delivery teams want to understand whether discharge referral activity progresses through an appropriate service pathway. Aggregate counts exist, but definitions and accountable next steps need to align.
Makes a shared operational problem inspectable without claiming that referral volume proves prevented readmissions.
What would we need to understand?
- Defined discharge referral cohort
- Aggregate service-stage records
- Reasons for incomplete progression
- Delivery effort and accountable next owner

Show uncertainty beside the count.
Useful outputs might include a stage-by-stage view with unlinked records shown separately, a definition comparison and a short list of questions for pathway owners. Completion proportions require a stated denominator and a defensible follow-up window. A fall in recorded referrals might indicate recording changes, eligibility changes or reduced activity; it cannot explain itself. Reviewers should be able to challenge every inference. The goal is a clearer improvement decision, not a visually impressive funnel that suggests causality the data cannot establish.
Agree what would invalidate the scan.
Stop if stage definitions cannot be reconciled, coverage is too incomplete to support the intended comparison or a draft presents an unsupported clinical outcome. Stop also if the preparation and checking effort exceed the value of the existing review process. A useful result may be that the pathway needs better recording before further analysis. That finding can redirect investment towards the actual constraint. Any later claim of reduced readmissions or cost would need an appropriate design, suitable data and independent clinical interpretation.

Build the improvement question together.
The right discussion brings hospital discharge pharmacy, community pharmacy representation and the commissioner or service owner into the same frame. Start with the question the next review meeting needs to resolve. We can discuss whether an aggregate scan is feasible, what a fair comparator looks like and what decision its findings could change. The purpose is to earn a focused evidence programme around a recognisable problem, with a clear option to stop if the information cannot support a useful conclusion.

Another useful angle.
- Homecare value is experienced between deliveries.
A homecare medicines service connects prescribing, pharmacy, logistics, nursing support where required and communication with the person receiving treatment.
- Make stewardship review easier to prepare, not easier to bypass.
An antimicrobial stewardship team needs information that supports careful review under real service pressures.
- The referral should arrive with a way forward.
A referral can be appropriate in principle and still be difficult to complete in practice.
