Commissioners & payers
A system-level opportunity needs a clear account of who does the work, who pays and when value could appear.
In development · Intended scope and questions for evaluation
The organisation.
The people around it.
An attractive total business case can leave a necessary provider without staff capacity or working capital. Those constraints need to be visible before a decision.
The people involved
Integrated care boards in England, relevant Scottish health-board and partnership teams, and jurisdiction-specific payers.
The dependencies
Providers, primary care, pharmacy contractors, finance, local government and the people using services.
What to examine
Affordability, payment timing, opportunity cost, inequalities, measurable outcomes and costs carried by each participant.
An illustrative set of programme relationships. The actual participants, authority, funding and permissions must be established for the defined setting. Explore every organisation perspective.
The decision worth examining.
Which programme or delivery arrangement should receive further assessment and, if authorised, funding?
An attractive total business case can leave a necessary provider without staff capacity or working capital. Those constraints need to be visible before a decision.
Develop the programme around the setting.
Begin with the local need, relevant evidence, existing work and an agreed comparison. A proposed programme needs clear activities, alternatives, resources and reasons to revise or reject it.
Affordability, payment timing, opportunity cost, inequalities, measurable outcomes and costs carried by each participant.
Give each participant a workable role.
Providers, primary care, pharmacy contractors, finance, local government and the people using services.
The overall case and the position of each necessary participant deserve separate examination. Staff time, cash timing, capacity, access, commitments and full costs can change which arrangement is practical.
Make the evaluation useful to a decision.
Identify an accountable sponsor, the current process, the permitted information and what a worthwhile finding would allow the organisation to decide. Include preparation, checking, correction, implementation and support in the comparison.
