The pharmacy day | A question to investigate

An order can finish before the care journey does

An online pharmacy can make access more convenient while still facing difficult questions about contact, suitability, supply and follow-up. A parcel delivered or a prescription processed is only one event in that journey. The operating challenge is to understand whether people can reach the right professional, whether unresolved issues remain visible and what happens when the standard route does not fit. Faster throughput alone cannot answer those questions.

Make the handovers visible

Identify each point where responsibility changes: an enquiry reaches the service, information is reviewed, a prescription is received, supply is arranged and an exception is resolved. Some organisations perform several of these functions; others depend on separate providers. The GPhC's distance-services guidance addresses the particular responsibilities of remote pharmacy provision (General Pharmaceutical Council, 2025). Our starting question is practical: can the team explain who owns the next step when the ordinary journey breaks down?

Separate prescription workflow from clinical decisions

EPS transfers prescription information between prescribing and dispensing services (NHS England Digital, n.d.). It does not remove the need for professional decisions or make every remote journey a single integrated clinical system. A proposed review would distinguish a missing prescription, an unresolved clinical query, a failed contact and a delivery exception. Combining them into a general delay measure can conceal which organisation is able to act and what information the patient needs next.

Look at the exceptions before scaling the average

A delivery success rate can look strong while a smaller group experiences repeated failed attempts or cannot use the contact channel provided. Examine repeat contacts, unresolved cases and the time to a clear next step. Keep ordinary operational delays separate from situations requiring professional escalation. The first analysis should use authorised aggregate records wherever possible; the public website does not require the details of individual prescriptions or sensitive customer journeys to explain the value of this work.

Review the unresolved case, not just the last contact

Track an exception from its first recorded occurrence to an explicit resolution or responsible handover. Count repeat contacts within that episode separately from distinct cases. Otherwise, a busy support team may appear to resolve many enquiries while the same underlying problem persists. An ageing view should identify the next accountable owner and the reason for delay, with clinical urgency managed through the provider's professional arrangements rather than a generic commercial priority score.

Own the next step when the standard route breaks.

A useful handover is information that reaches someone able to act. Explore a constructed case before deciding what to measure.

A remote order has an unresolved exception.

Makes a shared operational problem inspectable without claiming that referral volume proves prevented readmissions.

What would we need to understand?

  • Episodes entering the defined cohort
  • Agreed handovers and recorded progression
  • Elapsed time bands and reasons for waiting
  • Clarification effort and accountable next owner

A proposed continuity assessment

Choose one remote service pathway and a defined set of exception types. Compare the current handover process with a deliberately limited change, such as clearer ownership of unresolved non-clinical status queries. Inputs could include aggregate contact categories, elapsed times, team effort and recorded resolution outcomes. The comparator should cover similar demand and service availability. Clinical decisions would remain within the provider's professional governance, with any necessary data access and evaluation approvals agreed before work begins.

Put a cost around the whole exception

A delayed case can create several contacts across customer service, pharmacy and delivery teams. Count that work once across the journey, rather than presenting savings independently in each department. Distinguish reduced handling time from a smaller headcount requirement, and include the ongoing cost of maintaining any new process. Improved continuity may first appear as fewer repeat contacts or better case ownership, not an immediate reduction in operating expenditure.

Decide what makes an intervention unacceptable

A change should stop if it makes professional contact harder, closes cases without a justified resolution or obscures which organisation is responsible. Review patient choice and communication needs alongside operational measures. A polished automated answer is not a satisfactory result when the underlying question remains unresolved. PHARMORIS is being developed around accountable medicines decisions; any proposed remote use case must demonstrate that it supports, rather than displaces, appropriate professional review.

Bring the case type you cannot resolve cleanly

A useful first conversation begins with a recurring exception category and the handover where ownership becomes unclear. There is no need to disclose product architecture or patient-level data to establish that question. We can examine whether a bounded evaluation could make the process more intelligible, what capability would be needed and what evidence would justify continuing. The commercial opportunity is a service people can rely on when the standard journey stops being standard.
