Different systems. Useful questions. | A question to investigate
US Medicaid programmes | Read the state, the programme and the data
Medicaid is a useful example of why a national chart needs a local explanation. A public dataset can show reimbursed activity, but a decision about a service or purchasing process belongs to a particular state, programme and accountable organisation. We want medicines intelligence to preserve those distinctions so a clear graph becomes the beginning of a better question rather than an answer it cannot support.

Find the accountable buyer
A state medicines team, managed-care organisation or authorised analytical partner could have a legitimate evaluation question. Their responsibilities and data access differ. The first brief would identify who is making the decision, which population and benefit are in scope, and whether the sponsor can commission the work. Interest from a provider does not confer authority to speak for a state programme or access its confidential arrangements.
Public data has a defined meaning
State Drug Utilization Data reports covered outpatient drug activity paid for by state Medicaid agencies, with fields including prescriptions and reimbursement (CMS, n.d.). It is not a clinical record of every medicine used. We would label the programme, period, units, revisions and exclusions before comparison. The purpose is to stop a visually striking difference between states being mistaken for evidence of inappropriate prescribing, better care or an immediately available saving.
A first question worth testing
We propose selecting one carefully defined data-quality or reporting question before attempting a broad market analysis. For example, can an authorised team reconcile a chosen product group across releases and explain why a trend changes? The sponsor would define product mappings, time window and acceptable uncertainty. Inputs could begin with public data and documentation, avoiding person-level information unless a later, separately governed study genuinely needs it.
Keep pricing concepts separate
Medicaid publishes pharmacy-pricing resources, including NADAC (CMS, n.d.). Those references would be displayed separately from reimbursement observations and any authorised programme-specific information. We would not subtract one public figure from another and call the remainder a state’s profit or waste. Confidential rebates, different units and reporting adjustments can change the interpretation. A useful review should show which comparisons are supported and which require information outside the public dataset.
A public decision brief | An illustrative example
Open the question.
Inspect the reasoning.
Choose a business question, then look beneath its claim. See what a public source supports, what depends on the local setting and what a useful next step could be.
A US-facing medicines business, analyst or research team deciding whether a public spending comparison is meaningful.
The same medicine. A different denominator.
A spending figure becomes useful when the reader knows the programme, period, unit and exclusions behind it. Reconciliation comes before an attractive price comparison.
A proposition to investigate
A comparable question before a bigger chart.
A public-data brief could identify whether two measures describe sufficiently similar activity to compare. The proposed value is a traceable inclusion decision and a clear definition of the measure. No current price, negotiated saving, purchasing recommendation or live dataset calculation is produced in this specimen.
The reconciliation task is constructed. No records have been joined or analysed in this example.
What the next brief would contain
A decision someone
can commission.
- The decision
- Whether a defined public-data comparison is valid enough to commission.
- The comparator
- A second explicitly defined source with comparable scope, units and period.
- The evidence
- Source editions, medicine mapping, population, units, exclusions and reproducible inclusion rules.
- Stop or redesign
- Exclude comparisons when the denominator or cost meaning cannot be reconciled; retain the exclusion reason.

Keep this example briefThe pilot deliverable
The initial output could be a reproducible data note, an accessible exploration and a short decision brief. The comparator would be the sponsor’s current analytical process using the same frozen extract. Measures would include reconciliation accuracy, source traceability, preparation effort and the proportion of questions left unresolved. We would preserve the extraction date and transformation rules so another analyst can inspect the result rather than trust a polished graphic.
The release bridge an analyst can actually inspect
A sample deliverable would follow one sponsor-defined product group between two frozen State Drug Utilization Data releases. It would list the original product identifiers and units, mapping changes, revised observations and the rows that remain unreconciled. The accompanying chart would distinguish a changed published observation from a changed grouping rule. An analyst could then explain whether a visible movement survives consistent definitions, rather than send a spending graph into a meeting without knowing why it changed.
A recurring review tied to the reporting release
The relevant cadence would follow the sponsor’s data-refresh and reporting timetable, with an additional review when a release or mapping changes materially. An authorised programme analyst would supply the reporting question, accepted grouping rules and the reconciliation tolerances. Acceptance would require another analyst to reproduce the bridge from the same frozen files and identify unresolved differences. A repeat commission would depend on the recurring reconciliation work being useful enough to fund; a single national chart would not establish that demand.
Stop misleading comparisons early
A state ranking would be withheld if case mix, benefit arrangements or dataset coverage make it misleading. The study would pause if product units cannot be reconciled, material revisions are unexplained or the proposed use exceeds the sponsor’s authority. No clinical inference would be drawn from reimbursement alone. Where a question cannot be answered with public data, the result should say what additional evidence is needed and whether obtaining it is proportionate.
From public evidence to a funded discussion
The commercial opportunity would be a specific analytical or decision-preparation service that an authorised buyer cannot obtain more efficiently elsewhere. A later evaluation would need a clear deliverable and governance route. We would not treat every visible spending trend as a software market. The test is whether a recurring decision becomes more reliable and useful, with a buyer who values that improvement enough to support it.
